ARE RECEIVING SERVICES UNDER A MEDICALLY APPROVED PLAN OF CARE.
A. COMPLETES ASSESSMENTS IN A TIMELY MANNER AND IN ACCORDANCE WITH AGENCY POLICY.
B. DEVELOPS A WRITTEN PLAN TO PROVIDE INFORMATION AND DIRECTIONS TO OTHER HEALTH TEAM MEMBERS.
C. DOCUMENTS ALL FINDINGS, PLANS, INTERVENTIONS, AND CLIENT PROGRESS IN THE CLINICAL RECORD.
D. COMMUNICATES PLANS AND CHANGES TO THE PHYSICIAN AND/OR NURSING SUPERVISOR AND OTHER
AGENCY STAFF THROUGH THE CARE PLAN, WRITTEN PROGRESS NOTES, AND PARTICIPATION IN CARE CONFERENCES.
E. CONSULTS WITH THE MEDICAL SOCIAL WORKER AND/OR CLINICAL SUPERVISOR, OR CASE MANAGER, AS WELL AS, WITH
OTHER AGENCY STAFF, AS NEEDED.
2. COMPLETES SOCIAL HISTORIES ON ASSIGNED CLIENTS TO PROVIDE INFORMATION NEEDED TO DETERMINE LEVEL OF
SERVICE AND TYPES OF RESOURCES, TAKING INTO CONSIDERATION:
A. FAMILY RELATIONSHIPS AND SUPPORT SYSTEMS;
B. MEANING OF ILLNESS TO THE CLIENT AND HIS/HER FAMILY;
C. CULTURAL ATTITUDES IN RELATIONSHIP TO ILLNESS;
D. MEANING OF “HOME” TO THE CLIENT;
E. ABILITY OF CLIENT AND FAMILY TO ADJUST TO THE CARE OF THE CLIENT IN THE HOME;
F. THE FINANCIAL IMPACT OF CLIENT’S ILLNESS UPON THE FAMILY
3. DEMONSTRATES TEAMWORK AND EFFECTIVE COMMUNICATION TO ACCOMPLISH CLIENT, TEAM AND AGENCY GOALS.
A. PARTICIPATES IN CARE CONFERENCES AND OTHER TEAM OR AGENCY MEETINGS. SHARES CLIENT OBSERVATIONS AS
WELL AS PROFESSIONAL EXPERTISE. INITIATES INTERDISCIPLINARY REFERRALS.
B. SHARES INFORMATION ABOUT THE AVAILABILITY OF COMMUNITY RESOURCES AND HOW TO ACCESS INFORMATION AND
SERVICES TO FACILITATE CLIENT CARE AND FOR STAFF DEVELOPMENT, AS REQUESTED.
C. EDUCATES TEAM MEMBERS AND CLIENTS/FAMILIES ABOUT COMMUNITY RESOURCE AND HOW TO ACCESS THEM
APPROPRIATELY.
D. PARTICIPATES IN AGENCY QUALITY IMPROVEMENT ACTIVITIES.
4. PERFORMS JOB IN COMPLIANCE WITH AGENCY POLICIES AND PROCEDURES, AND PROFESSIONAL AND COMMUNITY
